From the 2000 or so Risk Influencing Factors (RIFs) that have been identified, HEB has found about 400 that are relatively common and troublesome. We have found it helps to group them into the 'families' illustrated in this table. You might want to consider how many of the issues indicated are being addressed effectively in your organisation.
