  Q5 Are people that make errors incompetent? 

 

If people have not been trained and assessed appropriately to assure their competence then clearly there is an avoidable risk that they will make an error. However, even highly competent individuals can and do make plenty of errors because of influences beyond their immediate control. We should also recognise that nobody knows everything and so even though somebody might have a high level of competence in some areas, in others they may be less competent.
