 Q1 What do you mean by ‘human error’?

 

An action or failure to act resulting in an unwanted outcome.  In other words, when something goes wrong that you didn’t mean to happen. A distinction can be made between an 'error of intention' (i.e. not realising that a weakness in your plan might produce the wrong result) and an 'error of execution' (your plan was OK but something went wrong when it was being carried out).
